Summary of work
The health services social worker coordinates medical and behavioral health services to clients in different programs. The Health Services Social Worker will work closely with IRC social workers and medical providers to coordinate all medical services for new arrivals. The Health Services Social Worker will develop partnerships with community organizations and ensure access to quality, culturally and linguistically appropriate physical and mental health services and health education for IRC clients. The Health Services Casework will provide health literacy training to clients to increase their self-reliance and ability to advocate for equitable medical services.
- Coordinate the provision of initial medical screening for all newly arrived refugees and asylum seekers.
- Coordinate the provision of medical case management services for clients enrolled in programs with medical and other service providers within the community.
- Outreach to local medical providers to improve health care options and provision for refugee and asylum seeker clients.
- Coordinate with the health team to arrange medical appointments for new arrivals requiring immediate medical assistance.
- Coordinate with health team and other program supervisors to provide client-centered services.
- Provide transportation or coordinate transportation for clients to appointments.
- Provide expert assistance and advocacy to clients needing help accessing health care coverage that may not be covered by Medicaid.
- Ensure proper documentation, records and tracking tools for case files and program reports. Create tools as needed for staff and perform regular file audits.
- Participate in the monitoring and evaluation of programs within the Department of Resettlement Cases.
- Monitor ongoing progress of outcomes for clients with assigned social workers, randomly meeting with clients monthly to review their cases.
- Provide case coordination and staffing for high-intensity cases and clients with significant vulnerabilities.
- Provide health literacy and health education sessions to clients.
- Provide recommendations for the improvement of programs and services.
- Comply with all IRC policies, procedures and protocols, including compliance with the IRC Way and child protection.
- Other assigned tasks.
Key Working Relationships
The position reports to: Health Program Supervisor
Education: Undergraduate degree in public health, social work or related field preferred.
- Relevant professional experience in the field of personal services required; Minimum 2 years experience highly preferred.
- Experience working with refugee and/or immigrant populations strongly preferred.
Demonstrated skills and competencies:
- Fluency in written and spoken English.
- Fluency in another language preferred.
- Must possess or be eligible to receive a fingerprint authorization card.
- Ability to prepare accurate and timely reports.
- Ability to document client interactions and services provided and maintain up-to-date and accurate case files.
- Proficiency in the Microsoft Office suite; Database experience strongly preferred.
- Demonstrated ability to work in a team in a multicultural environment.
- Valid driver’s license, reliable vehicle with current insurance and ability to travel regularly throughout the service delivery area (home visits, branch visits, service provider visits, etc.).
- Per Arizona State law, must possess or be eligible to receive a fingerprint authorization card.
- A combination of standard office environment, remote work and time in the field in the service delivery area to perform the responsibilities outlined above.
- May require occasional weekend and/or evening work.
